29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year. a) 1125 East Point Road - On or about March 31, 2021, the employer had provided an OSHA 300A annual summary for calendar year 2018, but the annual summary was not completed in detail with 11 injuries not recorded onto the annual summary from the OSHA 300 Injury and Illness Log. The annual review conducted by the terminal manager also failed to identify missing information on the 2018 OSHA 300 Log with the following fields not captured; G, H, I, J, K, L, and M.
