29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. At the Reitz Crust Mixer: On or about March 26, 2021, employees assigned to clean the machine were not required to affix an energy isolating device, exposing employees to an amputation hazard. The Father's Table, L.L.C., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(i), which was contained in OSHA inspection number 1181942, citation number 1, item number 1 and was affirmed as a final order on January 26, 2017, with respect to a workplace located at 2100 Country Club Road, Sanford, FL 32771.
