29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On or about March 22, 2021, employees were exposed to a fall of approximately 19 feet while performing roofing activities on a commercial building located at 2377 Medina Road, Medina, Ohio 44256. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection. Kirby Vanfossen DBA Vans Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.501(b)(13), which was contained in OSHA inspection number 1486208, citation number 1, item number 2, and was affirmed as a final order on 10/15/20, with respect to a workplace located at 635 Canal St., Canal Fulton, Ohio 44614.
