29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The lockout/tagout procedures developed by the company were not provided or utilized to control hazardous energy within the machine. (a) Location - 127 Rogers Street, Gloucester, MA 01930 - Line 4 of the packaging/manufacturing area of the facility. On or about 1/05/2021, an employee sustained an amputation while troubleshooting a Shick Rotary Air Lock Valve on a packaging line where the lockout/tagout procedures developed by the company were not provided or utilized to control hazardous pneumatic and electrical energy. Gorton's Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1217751, citation number, 1, item number 1 and was affirmed as a final order on 4/21/2017, with respect to a workplace located at 128 Rogers Street, Gloucester, MA 01930.
