29 CFR 1910.106(c)(7): Before being covered, enclosed, or placed in use, all piping was not hydrostatically tested to 150 percent of the maximum anticipated pressure of the system, or pneumatically tested to 110 percent of the maximum anticipated pressure of the system, but not less than 5 pounds per square inch gauge at the highest point of the system. Test was not maintained for a sufficient time to complete visual inspection of all joints and connections, but for at least 10 minutes: (a) On or about February 27, 2021 in the process area around the Stripper Column in Unit 6 at the Lion Oil Refinery in El Dorado, AR, workers were exposed to fire and explosion hazard when piping installed to charge the Unit 6 Stripper column with Naphtha from Unit 9 was not hydrostatically tested prior to being placed in use.
