Ventria Bioscience, 2718 Industrial Street, JUNCTION CITY, KS 66441 on or about 2/23/21, the employer failed to provide copies of the injury and illness records to an authorized representative within four hours. The OSHA 300 and 300A logs were provided on 2/27/21.
