29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) 136 Thorn St, Clarksburg: On or about January 10, 2020, an employee tested positive for COVID-19 after being exposed through a client who tested positive for the virus. The work related injury was not recorded on the employer's OSHA 300 log. b) 136 Thorn St, Clarksburg: On or about January 14, 2020, an employee tested positive for COVID-19 after being exposed through a client who tested positive for the virus. The work related injury was not recorded on the employer's OSHA 300 log. c) 136 Thorn St, Clarksburg; On or about January 15, 2020, an employee tested positive for COVID-19 after being exposed through a client who tested positive for the virus. The work related injury was not recorded on the employer's OSHA 300 log. d) 136 Thorn St, Clarkburg: On or about April 28th, 2020, the employer learned that an employee was diagnosed as positive for COVID-19 and died after being exposed through a client who tested positive for the virus. The work related death was not recorded on the employer's OSHA 300 log after being notified of the COVID injury.
