29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) McKinley Health Care Center located at 800 Market Avenue North. Canton, OH 44702: On or about December 5, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer was notified of the fatality on December 7, 2020. b) McKinley Health Care Center located at 800 Market Avenue North. Canton, OH 44702: On or about December 29, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours.
