29 CFR 1904.4(a):The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Eger Health Care and Rehabilitation Center, 140 Meisner Ave, Staten Island, NY On or about December 1, 2020 and thereafter, the employer did not record a work-related illness of an employee on the OSHA 300 log or its equivalent. The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19
