29 CFR 1904.39(a)(1):Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. (a) Facility- 902 Western Avenue, Beaver, PA: On or about February 1, 2021, the employer failed to report a work-related fatality within eight-hours to the local OSHA Pittsburgh Area Office.
