29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about November 19, 2020 the employer failed to report a work related amputation. An employee was injured on November 18, 2020 which resulted in an amputation of the finger. The employer did not contact and report the injury within 24 hours of knowledge of the event.
