29 CFR 1904.40(a):The employer did not provide an authorized government representative the records within the four business hours: (a)Roosevelt Rehabilitation & Healthcare Center located at 7800 Bustleton Avenue, Philadelphia PA: On or about January 25, 2021 the employer failed to provide copies of the OSHA 300 injury and illness logs or it's equivalent for the year 2021. The OSHA 300 logs for 2021 were provided on May 12, 2021 and have recordable illnesses.
