29 CFR 1904.7(b)(3):How do I record a work-related injury or illness that results in days away from work? When an injury or illness involves one or more days away from work, you must record the injury or illness on the OSHA 300 Log with a check mark in the space for cases involving days away and an entry of the number of calendar days away from work in the number of days column. If the employee is out for an extended period of time, you must enter an estimate of the days that the employee will be away, and update the day count when the actual number of days is known. On or about January 26, 2021, the employer did not correctly enter the following work-related injuries or illnesses on the OSHA 300 form for the calendar year 2020; (a) On December 2, 2020, an employee working as an order picker on the pack/ship line experienced back pain after lifting a heavy box. The employee received four days away from work and five days of restricted duty, but the numbers of days were not recorded in columns (K) and (L) on the OSHA 300 Log for the calendar year 2020. (b) On November 28, 2020, an employee working as a bulk seasonal picker at the bulk shipping station east dock experienced a contusion to the left toe causing nail damage after a box fell on their foot. The employee received five days away from work and ten days of restricted duty, but the numbers of days were not recorded in columns (K) and (L) on the OSHA 300 Log for the calendar year 2020.
