29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. On or about January 14, 2021, the employer did not use and maintain OSHA Form 300 "Log of Work-Related Injuries and Illnesses" and OSHA Form 300A "Summary of Work-Related Injuries and Illnesses" for recordable injuries in the workplace. The Illinois Form 45 "Employer's First Report of Injury" was maintained by the employer, but this document is not a substitute replacement for OSHA Form 300 "Log of Work-Related Injuries and Illnesses", as it does not contain all required information. There is no abatement documentation or certification required for this item.
