29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours. a) Park Ridge Care Center, Ltd. located at 665 Busse Highway, Park Ridge, IL 60068. On January 12, 2021, upon request, the employer did not provide the OSHA Form 300, Log of Work-Related Injuries and Illnesses, for calendar year 2020 and the Form 300-A, Summary of Work-Related Injuries and Illnesses, and Form 301, Injury and Illness Report, for calendar years 2019 and 2020, within four business hours. The OSHA 300 Logs, 301s and 300-A were not provided until April 23, 2021.
