29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: Atlantic Health System dba Chilton Medical Center, 97 West Parkway, Pompton Plains, NJ 07444. The employer did not record cases with days away from work for employees diagnosed with COVID-19 related illness and lost work time on the OSHA 300 Log for calendar year 2021 to include but not limited to: a) On or about 01/28/2021 the employer failed to record the COVID-19 positive illness of a Registered Nurse who was out of work due to COVID-19 on 1/22/2021. b) On or about 03/26/2021 the employer failed to record the COVID-19 positive illness of a Secretary/Nursing Assistant who was out of work due to COVID-19 on 03/20/2021. c) On or about 04/02/2021 the employer failed to record the COVID-19 positive illness of a Patient Care Technician who was out of work due to COVID-19 on 03/26/2021. Violation occurred on or about 1/28/2021, and thereafter.
