29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 28, 2020, an employee was exposed to a cave in hazard while performing waterline repair at 5446 Hollywood, Maple Heights, Ohio 44137. The employee was working in a trench that was approximately 5.5 feet deep and the employer did not ensure that he was using cave in protection. Catts Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1466647, citation number 1 item number 1 and was affirmed as a final order on 3/27/20, with respect to a workplace located at Wildflower Drive, Rocky River, Ohio 44116. Catts Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1405983, citation number 2 item number 2 and was affirmed as a final order on 7/17/19, with respect to a workplace located at 929 Pearl, Brunswick, Ohio 44212.
