29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Patterson Middle Spice Mixer Machine, SN: CA9777, Company number IS/ID#MXR-MIX01-04: On or about October 27, 2020 , the employer exposed employees to mechanical, and electrical hazards by not providing an energy control procedure to lock out/tag out the spice mixer machine. b) Day Spice Mixer Machine, Company number IS/ID#MXR-MIX03-04: On or about October 27, 2020, the employer exposed employees to mechanical, pneumatic and electrical hazards by not providing an energy control procedure to lock out/tag the Spice mixer machine. c) Spice Mixer Machine, Company Number IS/ID#MXR-MIX02-04: On or about October 27, 2020 , the employer exposed employees to mechanical, pneumatic and electrical hazards by not providing an energy control procedure to lock out/tag the spice mixer machine. iSpice Foods Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1302493, citation number 1 item number 2a and was affirmed as a final order on 7/18/2018, with respect to a workplace located at 6450 US 43 Jackson, AL 36545.
