29 CFR 1910.304(g)(5):The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: a. On or about October 15, 2020, and at times prior, in the Press Department by the Stokes R4 Part-Revolution Mechanical Power Press (MPP), employees were exposed to electrical shock hazards associated with an overhead light electrical cord that was missing the grounding prong. Burley Clay Products Company, Inc. was previously cited for a Serious violation of this Occupational Safety and Health Standard 29 CFR 1910.304(g)(5), which was contained in OSHA inspection number 1136715, citation number 1, item number 1, and was affirmed as a final order on October 4, 2016, with respect to a workplace located at 455 Gordon Street, Roseville, OH 43777.
