29 CFR 1926.1203(e)(2)(iii): Before an employee enters the space, the internal atmosphere must be tested with a calibrated, direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. On or about October 12, 2020, and at times prior thereto, employees were working in a sewer line manhole, a permit required confined space, without testing/monitoring the internal atmosphere of the space, exposing the employees to hazards such as, but not limited to, flammable gases, oxygen deficient atmosphere, or toxic air contaminants.
