29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a. Located at 4955 Carroll Eastern Road in Carroll, Ohio: On or about October 8, 2020, the employer did not ensure that personal fall arrest system anchorages were installed as approved by the equipment manufacturer as the anchors were secured on top of approximately four inches of insulation board. The anchorages were not imbedded in the substrate material and fastened directly to the metal deck, thereby exposing employees to a fall hazard in the event a free fall were to occur.
