29 CFR 1926.1432(b)(1): The employer did not ensure that multiple-crane/derrick lifts were directed by a person who met the criteria for both a competent person and a qualified person, or by a competent person who was assisted by one or more qualified persons (lift director): (a) On or about October 13, 2020 and times prior, the employer did not ensure that the multiple-piece lift of a 296 foot pipestring being crossed under an unprotected, live, natural gas line was directed by a competent and qualified Lift Director. A Competent & Qualified Lift Director was onsite at the time, however was performing the duties of an excavator operator on the eastern end of the pipestring and unable to identify and control the hazards during pipeline construction, in the vicinity of State Highway 5 & 52nd Street NW near Williston, North Dakota.
