29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about 9/24/2020 the employer had not recorded work related injuries/illnesses on the OSHA 300 log: a) Encapsulation Department: On 9/17/2020 an employee was hospitalized and sustained a finger amputation on the left hand while operating the Capsule Filling Machine Model NJP-1200 Encapsulator. b) Blending Department: On 9/17/2020, an employee was hospitalized and sustained a finger amputation on the right hand during assembly of the Dry Granulator Model GR-100p Chilsonator. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
