29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) K & K Surplus, Inc., 8381 Brighton Rd., Commerce City, CO 80022: On or before September 9, 2020 the employer did not certify with the manufacturer that a chain and clamp attachment was compatible with the forklift it was being used on. This condition potentially exposes employees to struck-by and crush-by hazards. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
