Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about August 31, 2020 at a facility located at 50 W. Fernau Avenue, Oshkosh, WI 54901; an employee was exposed to machine hazards associated with rotating parts when they cleared a jam on a Blower Application Co. Trim Cutter (Model 12SC S/N 070610A1). The employer did not ensure that employees implemented energy control application steps prior to allowing them to clear jams. The employee removed the out-feed elbow and reached their hand into the trim cutter to clear the jam. The trim cutter was turned off but not deenergized as the cutter head was still spinning prior to the employee engaging in servicing/maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. The Westgate Cups (dba Westgate Products) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1354135, citation number 1, item number 2 and was affirmed as a final order on April 03, 2019, with respect to a workplace located at 50 W. Fernau Ave, Oshkosh, WI 54901.
