29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Western Bee Supplies, Inc., 5 Ninth Avenue East, Polson, MT 59860: On or about August 6, 2020, and at times prior, employees were exposed to unexpected hazardous energy from rotating blades while cleaning equipment including, but not limited to, the Paulson Enterprises, Inc. Bottom Gang Rip Saw Model SL-10 Serial #63. The employer failed to develop, document and utilize procedures to effectively control hazardous energy for employees that clean the rip saw. One employee suffered injuries which resulted in a finger amputation when performing cleaning operations on the piece of equipment which was thought to be de-energized.
