29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Nursing Department - 3220 Henry Hudson Pkwy., Bronx, NY A Nursing Department Certified Nursing Aide died of SARS-CoV2 Coronavirus Disease on April 12, 2020. The employee cared for suspect and confirmed COVID-19 positive patients in March 2020. The employer did not record a COVID related illness on the OSHA Form 300. Violation occurred on or about 4/12/2020. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c).
