29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. Location: a) First Floor a) On or about 08/03/2020, Employees exposed to a fall hazard from the first floor to the basemen from unguarded floor openings. An employee was injured when he fell approximately 15 ft. Alba Services Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(i), which was contained in OSHA inspection number 1172069, citation number 1, item number 1 and was affirmed as a final order on 07/17/2017, with respect to a workplace located at 39 East 72nd Street, New York, NY 10021.
