1910.147(c)(7)(iv): The employer did not certify that the affected employee training had been accomplished and kept up to date. The certification shall contain each employee' name and dates of training: a. On or about July 11, 2020, employees in the packaging area, were exposed to amputation and laceration hazards when not provided with training to recognize applicable hazardous energy sources and methods and means for energy isolation when performing tasks including, but not limited to, unjamming equipment. b. On or about October 31, 2020, employees in the packaging area, were exposed to amputation and lacerations hazards when not provided with training to recognize applicable hazardous energy sources and methods and means for energy isolation when performing tasks including, but not limited to, unjamming equipment. Vee Pak, LLC dba Voyant Beauty was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i), which was contained in OSHA inspection number 1415105, citation number 1, item number 3, and was affirmed as a final order on November 14, 2019, with respect to a workplace located at 5331 Dansher Road, Countryside, IL 60525.
