29 CFR 1904.39(a)(1): Within eight (8) hours after the death of an employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor: a) Kennicott CILA Home located at 235 S. Kennicott Avenue, Arlington Heights, IL, 60005: On or about June 18, 2020, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8 hours. The employer reported the death to OSHA on July 10, 2020. No abatement certification or documentation is required for this item.
