29 CFR 1926.502(b)(2)(i): Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used between posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide. a. On or about June 29, 2020, at the above addressed jobsite, the employees onsite erected a mid-rail that was less then half the height between the top-rail and the walking working surface, on the sixth (6) floor hall, next to the elevator shaft.
