29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At a work place located at 4092 County Road 140, Seymour, Texas; an employee sustained arc flash burns while working on an energized cabinet of 12,000 volts. The employer did not report the accident to the Occupational Safety and Health Administration within 24 hours.
