29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent: (a) Location - 191 Foster Street, Littleton, MA: On or about 06/11/2020, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: a) From the time period between 03/20/2020 to 05/15/2020, five (5) incidents of COVID-19 infection resulted in days away from work. These five incidents include, but are not limited to, worker illnesses that involved between 2 days of work and 114 days away from work.
