Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Employees were not required to: (1) turn off and shut down the machine (2) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (3) affix lockout or tagout devices to each energy isolating device (4) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished. a) On or about June 5, 2020 on the MOX compound machine, employees, such as operators performing troubleshooting, and cleaning of the needle did not lockout the electrical and pneumatic disconnects when making part adjustments within the guard of the machine. b) On or about June 10, 2020 on the MOX, MO7 and MO8 Compound machines employees, such as operators performing servicing of the needle did not lockout the electrical and pneumatic disconnects when cleaning the needle within the guard of the machine. The West Troy, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.147(d), which was contained in OSHA inspection number 1232427, citation number 1, item number 3 and was affirmed as a final order on June 28, 2017, with respect to a workplace located at 650 Olympic Dr. Troy, Ohio 45373.
