29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. A work related incident which an employee was hospitalized was not reported to OSHA within 24 hrs at the Grainfield, KS location. The incident occurred on 05/22/20 at around 1545 and reported to OSHA on 05/26/20 at around 1530.
