FACILITY-WIDE: On or about May 26, 2020, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: a) From the time period between May 04, 2020 and May 19, 2020, there were three (3) incidents of COVID-19 infection resulting in days away from work and one fatality. Lost work days ranged from 11 and 16 days.
