29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. On or about 7/29/2020 the employer had not recorded work related injuries/illnesses on the OSHA 300 log: a) Unit 2: On 4/3/2020 a CNA providing care for a COVID-19 positive resident contracted COVID-19 and was out of work for 44 days. b) Unit 3: On 4/6/2020 a CNA providing care for COVID-19 positive residents contracted COVID-19. c) Unit 3: On 3/31/2020 a CNA providing care for COVID-19 positive residents contracted COVID-19 and remains out of work.
