FACILITY-WIDE: On or about August 13, 2020, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: a) From the time period between April 18, 2020 and July 15, 2020, seventeen (17) incidents of COVID-19 infection resulting in days away from work, an unknown number of hospitalizations, and one fatality.
