29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At 4115 S. Johnson Rd, Chanute, KS in the Fab Shop, A work related incident which in an employee hospitalization was not reported to OSHA within 24 hours. The incident occurred on or about 04/30/2020 at around 2:00 pm was reported to OSHA on 05/12/2020 1:01 pm by a secondary source.
