29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. (a) Facility wide: On or about 6/9/20 the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2020: From time period between 4/7/20 and 6/4/20 there were thirty eight (38) incidents of COVID-19 infection resulting in days away from work. These 38 incidents include, but are not limited to, workers illness that involved between 4 days away from work and 88 days away from work, and one incident of COVID-19 infection resulting in illness on 4-14-20 and death on 4-25-20.
