29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. a) 250 Bloomfield Avenue, Bloomfield, NJ. On or about April 23, 2020, the employer was notified by the Bloomfield Health Department of the COVID-19 death of a Patient Care Assistant in their employment. The employer failed to notify OSHA of the fatality until April 24, 2020.
