29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about April 14, 2017, an employee was hospitalized for a severe and disabling injury to the second and third digits of the left hand and the employer failed to report the hospitalization within 24-hours. The employer reported the hospitalization on April 17, 2020.
