29 CFR 1904.4(a): The employer did not record each work-related fatality, injury, and illness case that met the general recording criteria on the OSHA Form 300 or equivalent: a) Emergency Department, 111 Central Ave., Newark, NJ: On April 9, 2020, a registered nurse from the Emergency Department died from COVID-19. The employer did not record the workplace illness and fatality on the OSHA 300 Log for calendar year 2020. Violation occurred on or about 4/9/2020.
