29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about 10 April 2020, the employer failed to perform and document a written certificate of an evaluation of the workplace pertaining to a hazard assessment. Employees are required to work with machinery and chemicals, in a general industry setting. The employer failed to assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). Employees were thereby potentially exposed to hazards in the work place. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(c). Pearl Valley Farms, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(2), which was contained in OSHA inspection number 1199638, citation number 2, item number 1 and was affirmed as a final order on 03/09/2017, with respect to a workplace located at 968 S. Kent Rd, Pearl City, IL. 61062.
