29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) PB-15 "Dino" Svartek grinder shovel: On or about April 7, 2020 and at times prior thereto the employer had not developed, documented and utilized specific energy control procedures for scheduled and unscheduled maintenance on the PB-15 grinder shovel cylinder and guide rods. The cylinder and guide rods use pneumatic pressure set between 90 to 100 pounds per square inch. Note) Certification and documentation of corrective actions are required for this item.
