29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) 3x3 Roll former cutoff - On or about 3/28/2020, the employer allowed employees to perform work on hazardous machinery which required the bypassing of guards and placing the employees body within the danger zone of the machine without utilizing lockout procedures, exposing employees to amputation hazards.
