29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about February 4, 2020, the employer did not report to OSHA within 24 hours or any time thereafter the work-related amputation injury of an employee on February 4, 2020 (see Case No. 0520 on 2020 OSHA 300 Log). b. On or about November 4, 2019, the employer did not report to OSHA within 24 hours or any time thereafter the work-related amputation injury of an employee on November 4, 2019 (see Case No. 1919 on 2019 OSHA 300 Log).
