29 CFR 1910.119(j)(4)(iv): The employer did not document each inspection and test that had been performed on process equipment. The documentation did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test. a.) Combined Systems, Inc.- On or about February 14, 2020, and at times prior thereto: employees were potentially exposed to fire and/or explosion hazards in that, the employer had not documented results of inspections and tests performed on process equipment including, but not limited to, loaders #1,#2, #3, and fill-pro. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
