29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: Russell T Bundy Associates, Inc. - Mansfield, OH: On or about February 11, 2020, the employer did not conduct an evaluation to determine the respiratory hazards associated with working at the tank line. Employees were potentially exposed to hazards associated with diethylene glycol, 2-methylpentane-2,4-diol, potassium hydroxide, ethylene glycol phenyl ether, sodium metasilicate, and sodium tetraborate, such as, but not limited to, burns to the skin and eyes, irritation to the nasal and respiratory tract, eyes, throat, and skin.
