29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. At 10825 Tom Folsom road in Thonotosassa, FL, on or about February 11, 2020, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out or incorrectly filled for the calendar year 2020: a. On or about January 29, 2020, for case number A2, an employee was attaching an anchor on the roof in new residential home located in 1679 Via Lago Drive in Lakeland, FL, when he slipped and fell off the roof, resulting in an in-patient hospitalization. The worker's compensation form used by employer as an equivalent to the OSHA form 301, did not contain if the employee was treated in the emergency room, if the employee was hospitalized as an in-patient, case number from the OSHA 300 log and time employee began to work on the day of the incident. b. On or about January 7, 2020, for case number A1, an employee was cutting duct tape with a utility knife at a site located in 2333 Hollow Forest Court in Wesley Chapel, FL when he cut his left palm. The worker's compensation form used by employer as an equivalent to the OSHA Form 301, did not contain if the employee was treated in the emergency room, if the employee was hospitalized as an in-patient, case number from the OSHA 300 log and time employee began to work on the day of the incident.
